| 内容記述 | 前大脳動脈水平部(A1)及び中大脳動脈水平部(M1)脳動脈瘤は比較的に稀である.過去15年間に経験した13例の同部位動脈瘤の死亡率,有病率はそれぞれ15%, 38%と他部位の動脈瘤に比較して高値であった.同部位動脈瘤の特徴として,多発性が多く(46%),又,出血した場合脳内血腫を高頻度(33%)に合併することが挙げられる.13例中11例にclippingが行われたが,clippingは穿通枝を温存して終了することはしばしば困難で,特にM1部のものは術中温存したと考えても術後閉塞を示すことがあり,4例に術後穿通枝領域に新たな梗塞巣を認めた. | |||||||||||||||||||
